How will I know that adult day services are the right fit for my loved one?
At Autumn Leaves, we offer same care you provide in the home – assistance with walking or transferring, eating, using the toilet and/or shower, and taking medications on schedule. Autumn Leaves’ nursing staff can also provide some medical care necessary to meet each guest’s needs.
We provide a safe, contained space for guests with memory or cognitive impairments. Our staff completes an assessment of each prospective guest prior to enrollment so that we can fully discuss and plan for your loved ones’ needs.
Do I have to pay for service if I need to cancel?
We understand that people become sick or otherwise have to cancel. Please contact the center as soon as possible if you know you won’t be attending. Canceling service by noon on the day before your scheduled day can be done at no charge. Cancellations made by 9 am on your scheduled day will be charged at 50% of the rate for the day(s) missed. Late cancellations and no-shows will be charged at the full rate for the day(s) missed.
Is there any financial assistance for this service?
The cost of adult day services is considerably less than other forms of care. Adult day is about one-fourth the cost of a nursing home and about one-half the cost of in-home care (long term care cost calculator). However, we understand that some people may not be able to afford our care.
There are several options that our staff can discuss with you. These include Medicaid Waiver services, Indiana’s CHOICE program, reimbursement from long-term care insurance, and a special scholarship fund provided by donors. Please talk to the Director about what options might be available to you.
How can I help support Autumn Leaves of Eastgate Adult Day Center?
Autumn Leaves operates as a ministry of Eastgate Christian Church, a registered charitable organization. You can donate online or order from or Amazon Wish List HERE.
We also offer many volunteer opportunities. If you have a particular skill, interest, or simply want to share some fellowship with an older adult at the center, please contact us.
Hours and Fees
.HOURS: Monday – Friday 7 am – 6pm
Open Two Saturdays a month
(1st and 3rd Saturday)
RATES: (effective Jan 1, 2022)
DEPOSIT: Varies (see below)
FULL DAY FEE: $90 per day*
HALF DAY FEE: $70 per day**
Enrollment deposit is equal to four weeks of fees based on your individual schedule
* FULL DAY FEE is up to 10 hours per day and includes snacks and lunch
**HALF DAY FEE is up to 5 hours per day and includes lunch and a snack.
There is a $20 charge for late pick up. Additional fees apply for shower service, massages, salon services and health care services (PT, OT, Podiatry, and Personal Wellness)
Guests may attend either Full or Half Days. The minimum attendance is 2 Half Days or 1 Full Day per week. Regular attendance helps guests develop familiarity with the program and staff.
